Who uses Home Health Care?
Persons discharged from the hospital, rehab, or nursing facility who are not fully recovered.
Persons with chronic conditions such as heart or kidney disease, stroke, wounds, or diabetes who require frequent monitoring.

What is Hospice Care?
Hospice emphasizes comfort rather than curative treatment; quality rather than quantity of life. Professional medical care is given, and symptom relief provided. Both the patient and family are included in the care plan. Emotional, spiritual and practical support is given based on the patient's wishes and family's needs.

Who qualifies for Hospices?
Hospice care is provided to patients who have a limited life expectancy. Although most hospice patients are cancer patients, hospices accept anyone regardless of age or type of illness. These patients have also made a decision to spend their last months at home or in a homelike setting.
